The sponsor in Saudi Arabia must apply for a visa authorization through the Ministry of Foreign Affairs. Once the application is approved, it will appear in the MOFA system. Medical Report
If you require a medical report, the report is valid for only Three months from the date of issue.
Please note that all tests on the Medical form must be completed. These medical tests are available in Canada and their completion is mandatory (few physicians may suggest that the testing is not needed, however it must be completed in order for the Saudi work visa application to be processed).
If the medical report is processed in Canada, it will take approximately one month.
If the medical report is processed outside of Canada, it has to be identical to the medical form at the bottom of this page and testified by the Saudi Arabian Embassy IN THE COUNTRY OF ISSUE.
You MUST confirm with your doctor that you have all required test results INCLUDING (HIV, Syphilis, Hepatitis B and C).
Authentication of medical report
The purpose of the attestation of the College of Physicians and Surgeons is to confirm that the physician is licensed in that province or in that territory. The College of physicians and Surgeons does not attest the test results.
Ensure that your Medical report you require to be authenticated by the College of Physicians and Surgeons is signed by the physician and that his/her registration number appears on the report. The name of the physician must be printed and clearly legible.
Please submit:

Two recent (No more than a month old) photographs (4×6 cm) on a plain white background (write name, signature and stamp date on back).
A valid passport for at least six months from the application date and must have the place of birth and two blank pages side by side (Original Only). If family accompanying on the same passport, pictures and names of all members must be added on the passport by the proper legal authorities.

Proof of relationship to the sponsor Submit only a copy with following policies:
Certificates issued from Canada or Arabic Countries for proof of relationship to the sponsor (a marriage certificate for the spouse, and a birth certificate for a child, brothers, sisters, indicating name of parents), must be notarized by a Canadian notary public or a commission of oat, and Authenticated by the Canadian Department of Foreign Affairs (Global Affairs Canada).
Non-Canadian Birth Certificate or Marriage Certificate if in English or French languages should be stamped from Canadian Foreign Affairs.
Non-Canadian Birth Certificate or Marriage Certificate in languages other than Arabic, English or French should be translated to Arabic, English or French and stamped from Canadian Foreign Affairs.
If Pakistani passport or Pakistani Notional ID card or Indian, Syrian , Palestinian, Sudanese, Egyptian passport or any official family book record must be:
I. Showing the relationship clearly (Full names of the father or mother).
II. A notarized copy by a Canadian notary public or a commission of oat, and Authenticated by the Canadian Department of Foreign Affairs (Global Affairs Canada).
If the document is a Saudi issued Birth, Marriage Certificates or copy of an old Igama (Saudi resident card) showing the relationship are accepted even if it is a copy.
Non-Canadian applicants must submit a notarized photocopy of their valid residency permit (Permanent Residence Card). Send only a copy of the front and back sides of the PR card. The copy must be notarized by a Notary Public and stamped by the Canadian Department of Foreign Affairs, International Trade and Development. The Notary must state on the copy that it’s a true copy of the original. (New Requirement)
Copy of Residency permit of the sponsor (IQAMA) and a Letter from the sponsor indicating the authorization number , the date and the names of applicants.
For residency visa applicants over 16 years old, must provide:
A Completely filled out medical report form that MUST BE:
Valid, less than Three months from the date of issue.
The physician used a CHECK MARK (✔) for POSITIVE RESULTS OR NEGATIVE RESULTS only.
The report certified by college of physicians of the issuing province (Original Only).
Copy of all required test results.
(HIV, Syphilis, Hepatitis B and C – please attach copy of these test results after the medical report right away).
If any of the above 4 results are positive +, then your Doctor must provide a typed explanation of why they are positive. On the other hand, If the results are expressed in a numerical value, then your Doctor must provide a typed explanation that the results are interpreted as Negative and that you do not have the disease. The Doctor’s note must be typed on the Clinic’s letterhead and stamped by their official seal or stamp as well as signed by the Doctor, with their license number appearing beside the signature.
If medical report is done outside of Canada, it must be authenticated by the KSA Embassy in that country.
An original valid Canadian Police Records Check. (Less than three months from the issuing date – you can obtain this certificate simply by NAME SEACH only and no need for fingerprint processing).

The KSA Embassy has advised that no ticket should be purchased before issuance of the visa.
The KSA Embassy in Ottawa can issue visas that allow travelers to enter KSA by air exclusively. No Entry By Land.
Please ensure that your entry airport is the closest to your sponsor’s location. The port of entry to KSA must be the same or the closest airport to the location of the sponsor. If the sponsor invited the applicant to conduct business/visit in a different city other than the one displayed in the company’s Registration Certificate, the sponsor must provide a letter in Arabic explaining the reason for this. An example can be for conducting business in a another branch/location of the company.
If applicant is female and 18 years of age or older, and going under her SPONSOR’s Iqama – she must submit a NOTRIZED DECLARATION LETTER from her PARENTS stating that she is single.
Female applicant (if applying without her husband) over 18 must have consent letter from her husband if she is married or notarized letter that she is not married.
